89321 is the code for the test that confirms your vasectomy worked. It's smaller and cheaper than the fertility semen analysis — here's what it covers and how to get it without going back to the urologist.
A post-vasectomy semen analysis (PVSA) only has to answer one question: are there any sperm left, and are any moving? CPT 89321 is that test. Hera's physician issues the order, you drop a sample at a lab near you, and the report says whether you can stop using backup contraception.

CPT 89321 is semen analysis for sperm presence and motility of sperm, if performed. The lab examines a fresh sample under the microscope and, if no sperm are seen, usually centrifuges it to look for rare ones. It reports 'no sperm seen', 'rare non-motile sperm', or a count with motility. It does not report morphology, volume or a full concentration — you don't need those after a vasectomy.
That's why it's cheaper than CPT 89320, the complete semen analysis. Some labs only offer the full analysis; that's fine clinically but you'll pay for parameters that don't matter. If a lab quotes you a price for a 'semen analysis', ask whether it's 89321 or 89320.

Many health plans cover the post-vasectomy check as part of the vasectomy itself when billed with Z30.8 and Z98.52 — ask your urologist's office whether it was included. The cash prices on Hera are for patients paying the lab directly, which is often cheaper than a follow-up visit.
