CPT 89320 is the standard, complete semen analysis: semen volume, sperm concentration and total count, motility, and a morphology differential. It's the code behind almost every 'semen analysis' or 'sperm test' a fertility clinic, urologist or hospital lab orders — and the one a Hera order requests.
Two neighbours matter. CPT 89322 is the same analysis using strict (Kruger) morphology criteria; most fertility clinics and andrology labs bill this one. CPT 89321 is sperm presence and motility only — the post-vasectomy check, not a fertility workup. If a lab quotes you a price, ask which of the three it's for.

Insurance coverage follows the diagnosis code, not the CPT code: Z31.41 is often covered inside an infertility workup, but many plans exclude fertility testing altogether. Hera shows each lab's cash price up front so there's no surprise bill.

Both are complete semen analyses. 89322 uses strict (Kruger) morphology criteria, which most fertility clinics and andrology labs report; 89320 is the general morphology differential. The sample, prep and other parameters are identical — the difference is how sperm shape is graded and how the lab bills.
The post-vasectomy semen analysis: it checks only whether sperm are present and whether any are moving. It's a smaller test than 89320 and shouldn't be used for a fertility workup, because it reports no count, morphology or volume.
Z31.41, encounter for fertility testing, for a first test. If infertility is already being worked up, physicians use N46.9 (male infertility, unspecified) or a specific code such as N46.01 (azoospermia) or N46.11 (oligospermia). For a post-vasectomy check the usual codes are Z30.8 and Z98.52.

Cash prices for a complete semen analysis run from about $75 to $300 in the US depending on the lab and city. Hera lists the exact price for each location and charges a $60 physician-order fee on top. Insurance rates vary, and many plans don't cover fertility testing.
