Leukocytospermia: white blood cells in the semen
Leukocytospermia — also called pyospermia — means your sample contained more than 1 million white blood cells per millilitre. It is one of the few semen analysis findings that points to a specific, often treatable cause rather than a vague production problem.
The round-cell trap
Many labs report "round cells" without distinguishing what they are. Round cells in semen include both white blood cells and immature sperm cells, and the two look similar under a basic microscope but mean completely different things — immature sperm cells are a normal finding.
Only a peroxidase stain or an equivalent test tells them apart. If your report says round cells are elevated but does not specify white blood cells, that is not yet a diagnosis of leukocytospermia. Ask for the specific test before accepting antibiotics.
What it means and how it is treated
White blood cells generate reactive oxygen species, and that oxidative stress damages sperm membranes and DNA. This is why leukocytospermia commonly appears alongside poor motility, and sometimes alongside reduced vitality or elevated DNA fragmentation.
The usual causes are infection or inflammation of the prostate, epididymis, or urethra, including sexually transmitted infections. Notably, many men have no symptoms at all — no pain, no discharge — which is why it turns up unexpectedly on a fertility workup.
Management starts with a semen culture and STI screening to identify an organism. Where one is found, targeted antibiotics are used; where none is found, anti-inflammatory treatment and antioxidants are common.
